Concerns

This form is for you to share your concerns regarding any aspect of Brereton Wheelers. No matter how big or small the concern we would like you to inform us about your concern.

Your concern will be kept confidential and not shared with anyone outside of Brereton Wheelers Committee. Your personal details will remain confidential and you will be informed of the outcome of your concern.

Incidents

This form is to be used when there is any kind of incident that the Club needs to be aware of.

• An accident whilst on a club ride
• A vehicle passing by dangerously
• An incident with a member of the public or another club member
• Any damage sustained to your bike or yourself
